Confined Space Entry Hazard Assessment

• Each confined space must be thoroughly assessed for hazards and risks. This is the first document an OHS officer will request.
• This template guides you through identifying the space and its physical attributes, documenting the reason for entry and planned work activities, cataloguing hazards - including hazardous energy and air quality, and determining the space's overall risk value.

Confined Space Entry Safe Work Plan

• A safe work plan must be written and communicated to the entry team (entrants, attendants, and entry supervisor) before entrants enter the confined space.
• This template covers the required pre-entry setup controls, gas testing locations and process, work materials, tools, and equipment, the number of entrants permitted in the space, and required safety controls and PPE.

Confined Space Entry Rescue Plan

• A rescue plan must be written and communicated to the entry team and the emergency response team.
• This template defines the rescue method (non-entry or entry rescue), required retrieval equipment, rescue team contact information, communication procedures, and response steps to follow in an emergency.

Confined Space Entry Log

• Each entrant's entry and exit time must be recorded by the attendant.
• This template provides a real-time headcount of all entrants inside the space in case of an emergency evacuation.

Atmosphere Gas Testing Log

• All gas testing inside the confined space must be recorded by the qualified gas tester.
• This template tracks oxygen, flammability, and toxic gas readings at set intervals to confirm the space remains safe to occupy.

Gas Detector Bump Test and Calibration Log

• The gas detector must be bump tested before each use, with results and calibration dates recorded.
• This template records the device model and serial number, gas calibration mixture and lot number, bump test date and result, and calibration date and result; confirming the instrument is reading accurately before it is relied on to detect a hazardous atmosphere.

Gas Information

• Key information on atmospheric hazards and gases present must be documented before entry.
• This template records details on the hazardous gases identified in the space, including exposure limits, flammability, physical properties, IDLH (immediately dangerous to life or health) value, and required PPE.
