Lockout Tagout Request Form

• Before any machinery, equipment, or system can be de-energized, the work must be formally requested and approved; usually by the maintenance department or a specialty contractor performing the isolation.
• This template captures the equipment being de-energized, the type of hazardous energy involved, the reason for de-energizing, affected departments or personnel, and the responsible parties (supervisor, person performing de-energization, person verifying zero energy); creating the paper trail an OHS officer expects to see before work starts.

Lockout Tagout De-Energization Plan

• Once the request is approved, this plan defines exactly how the isolation will be carried out. This is the document an OHS officer will review most closely, since it shows the isolation was planned in advance rather than improvised on the spot.
• This template identifies all the sources of energy (switches, circuits, valves) that must be isolated and locked, the sequence and procedures for isolation, methods to test for zero energy, and methods to control residual energy.
